    Keep in mind that the EPA is a tool to create policies, pushed by lobbyists, onto politicians, that will create rules or boundaries that will benefit their industry. That’s usually by banning or limiting another industry for something that’s not real.

    And where is Surgery/ObGyn when they are needed to perform a laparotomy? Every hospital that i know has a surgical department on call, just call the hospital before you arrive and they will be ready. Is it an US thing for the EM to perform amateur procedures instead of surgeons?

    They come out with nicotine pouches...nicotine is pretty safe except it causes horrible gum recession. They decided to use ACESULFAME Potassium as a sweetener ...it hasn't been tested enough to be considered "safe." Can enhance tumor growth and stomach issues (heartburn). These pouches would be safe if they didn't dump ACESULFAME into them! It's probably way worse than nicotine or tobacco. They get you coming ..and going. They need to add crap that'll kill you or cause tooth loss due to holding nicotine against your gums for hours.

    Here is an anecdote of the risks of nicotine pouches: I live in Norway, where nicotine pouches, and the similar tobacco pouches known as «Swedish snus» have been widespread among youths for decades. There are many brands on the Norwegian market, and the nicotine strength of these can be as high as 50mg per pouch, but most people thankfully stay below 20mg (which is definitely too much for me). Since the age of 17, I became more and more addicted to nicotine pouches, and after two years, I was going through roughly 12 pouches of 13mg/pouch nicotine strength daily, in addition to drinking around 4 (pretty big) cups of coffee daily. Then one day, as a 19-year-old man with no known health complications, I had an episode of an extremely high heartbeat. (I think it’s called tachycardia?) I measured my pulse at more than 200 BMP, and freaked out, calling the emergency number, fearing for my life. The lady at the emergency central told me it was not an emergency, and that I should go to the doctor (I never did 🙊). After that, I was much more cautious about my nicotine and caffeine consumotion, and it has never happened again. A while after this episode, I heard from a friend that she had also experienced the exact same thing, and at a party, I learned that this was a phenomenon that people knew about. I still have no idea how serious this is, or what exactly causes it, but it definitely lights up a red flag about nicotine consumption for me.

    Great video! But I must point out that you said that Zyns have less nicotine than cigarettes at 1:33 . This is technically true, but only around 1/10 of the nicotine in each cigarette can actually be absorbed in the body. The EU has a law banning cigarettes that have more than 1mg of nicotine, but that is measured in the absorbable «free nicotine». Nearly all of the nicotine in nicotine pouches can be absorbed, but it is released over a longer span of time, so the effects aren’t felt as strongly using a Zyn pouch as a cigarette.

    • @Kilzu1
      @Kilzu1 5 днів тому

      Nicotine, like all alcaloids actually gets into your body VERY easilly, in fact you get nicotine just by smelling it, which is why people who smoke cigars and cigarettes take a small sniff first, before litting a cigar or cigarette, since smell is only way to tell what things taste like and nicotine just happens to be one of those things, which makes you physically ill, if you take it mixed with flavors you don't like.

    • @Kilzu1
      @Kilzu1 5 днів тому

      Also you can't even compare nicotine pouches to smoking, since way nicotine gets to your body is different. When you smoke cigarettes, it takes a while for nicotine to have any effect, when you smoke cigar, it hits like a mule (especially if you inhale the smoke, since cigars are pure tobacco and don't contain chemicals, which slows down the whole nicotine effect) when you take nicotine pouches, it literally drills a hole into your gum and nicotine is released straight to brains which is why nicotine pouches can pack quite a punch. Now if you want to use nicotine pouches, go ahead, but don't fool yourself or anyone into thinking, that taking nicotine pouches has no health risks. Nicotine can cause increased blood pressure, heart problems among some effects on brain receptors and that's what too much nicotne does, basically it does something to your body, which leads to point where you can't breathe and suffocate. That is why it is next to impossible to accidentally take lethal dose of nicotine, since you get gag reflex, nausea, uncontrollable vomiting and pass out after taking not even half of the lethal dose of nicotine (which btw is about 1gram).

    I would warn anyone falsely advertising poison - Ks, Spice - as "synthetic cannabis" to cease immediately. Neither the manufacturer, nor the retailers, advertise, nor have they ever advertised that crap as synthetic cannabis or marijuana. Only the medical community, law community, and media advertise, and that to the stressed out masses desiring to use real cannabis, but have to piss for a job, that a scientific impossibility - man synthesize a natural plant, cannabis, or marijuana. Those alcoholic and codependent authorities are responsible for tempting millions to try K2 and Spice by advertising it as synthetic cannabis. Countless souls have been harmed. Lawsuits are coming for damages. You had better stop doing it yourself.

I wish more content discussed the risks and benefits of choices we make and is honest when the research just simply hasn't been thoroughly conducted yet. Do you have a reference for the 48 times more likely to be killed on a road bike compared to a car? A shocking number I think. I am definitely not on par with traffic accident numbers, but I would think that automobile related deaths and injury will be higher than cycling related.

    • @willswanson1840

      Also worth mentioning that people who cycle to work have increased life expectancy - even factoring in the increased risk of fatality via traffic violence - because of the health benefits of an active commute.

    Prior to beginning medical school, I worked in an ER as a tech. It was my responsibility to either apply the splint or assist in traction or counter traction with the physician. It is a skill that I am grateful to have learned and developed. I have an EM sub I coming up soon and this will be something I can do as a medical student that is actually helpful for the department staff. I hate being a med student that just stands by my preceptor or even worse in the corner and observes everything. I understand that it’s essential to not be in the way for codes, traumas or priority patients. I do find that being very proactive in my learning by asking the nursing staff to teach/guide me on other tasks like IV’s and catheters. They are often enthusiastic about helping and teaching. I can learn just as much from an experienced nurse as I can an attending. I also want to be a self reliant provider that is not above performing nursing skills or practice. I found that the most difficult reduction tended to either a hip or shoulder dislocation. Especially in young, strong fit individuals that unintentionally contracting and resisting our efforts. The attending often had to call the orthopedic service for some assistance. At times, they even required more than one attempt and in a few circumstances, had to go to the OR for an open reduction. Growing up in Minnesota, a snowy or icy day always produced a lot of falls (geriatric population) and distal radius fractures. I was encouraged and instructed by the ortho group that utilizing a finger trap and counter weight for Minimally displaced fractures worked excellent. Especially in patients that had an increased risk of complications following the administration of sedatives or dissociative medications. This technique resulted in muscular fatigue and relaxation that helped realign the fracture. It was often well tolerated by the patients and even helped hold their extremity in place for accurate measurements and acted like an extra hand to hold things in place when the department was blowing up and asking for assistance was not an option. From my personal experience, I found plaster to be a lot more forgiving and much easier to mold. Fiberless certainly has its value but I found it to be bulky and cumbersome. If you don’t trim the edges behind the cloth material they would splinter and harden into very sharp and jagged pieces. Plaster tended to result in greater comfort and tolerability for the patient. Splinting is certainly an art. It took me a fair amount of practice to understand how to measure appropriately and efficiently apply the splint in a time sensitive manner. Eventually I got to the point that I could make some very good looking splints and took some pride in my abilities. I completely agree with your emphasis on not applying too much pressure when attempting to mold the plaster. Applying too much pressure, resulted in patients experiencing more discomfort and would often return to the ER for removal and re-application. Which is not only an Inconvenience for both the patient and staff but also financially frustrating for the patient and budget. Near the end of my employment, the ortho group strongly recommended only using plaster and avoiding fiberglass altogether. The most challenging splint to apply was a long posterior leg and ankle stirrup for a Tib/fib fracture. Even more so when trying to use fluoroscopy for confirmation imaging. It’s no fault of the attending but when the help hold the materials and reduction in place, their hands are always in the way. It takes a fair amount of team work and communication to work efficiently. This is a great video and I found it to be a very helpful, thank you!
